MIRAI 3D

Liver tumour resection with 3D biomodels

Dr. Gustavo Nari

Hospital Tránsito Cáceres de Allende - Córdoba, Argentina

Benefits

Better anatomical understanding

Surgical plan design

Improved patient safety

Reduction of surgery time

Clinical case

A 65-year-old female patient with a history of surgery for a double primary ovarian and colorectal tumour and adjuvant treatment presented with liver metastases in the right lobe and a small one in the left.

In addition, her medical history indicated that she had congenital thrombocytopenia with values between 16,000 and 25,000 platelets (normal > 150,000), a condition for which it was of utmost importance to have rigorous control of the surgery in order to reduce major haemorrhages.

Surgical plan and results in operating room

Surgical treatment consisted of a right hepatectomy together with limited resection of the associated left lobe metastasis. The physical 3D model together with its virtual counterpart was used to design the approach to be followed during surgery.

The implementation of biomodels allowed us to have a comprehensive view of the problem by recognising the tumour with its associated vascularisation and the physiological hepatic vascularisation. In this particular case, having an accurate identification of the vascular structures was of great relevance due to the patient's pathology in relation to blood coagulation, giving confidence to the surgical team and greater security to the patient.

"The experience was very good and thanks to the model we went into the surgery with more confidence. The knowledge of the vascular anatomy and its relationship to the tumour facilitates the operative plan", says Dr. Nari about his experience of operating with 3D technology.

Thanks to the use of this 3D model, the surgical time could be reduced by approximately 45 minutes, resulting in a total surgery time of 3 hours.
